This role plays a critical part in ensuring accurate, timely, and compliant clinical documentation of patient encounters, wound assessments, procedures, and treatment plans within the electronic medical record (EMR).

The Medical Scribe partners closely with providers and clinical teams to improve documentation efficiency, support regulatory and quality reporting initiatives, and enable clinicians to focus on delivering exceptional patient care. The ideal candidate is highly organized, comfortable working independently, adaptable to changing priorities, and willing to provide both remote and onsite support as needed.

Key Responsibilities
  • Accurately document patient visits, wound assessments, procedures, and treatment plans under provider direction.
  • Provide real-time documentation support during onsite provider rounds and remote documentation support as assigned.
  • Ensure clinical documentation is complete, accurate, and entered into the EMR according to organizational standards, payer requirements, and regulatory guidelines.
  • Capture relevant clinical details including wound measurements, progression, interventions, treatment responses, and follow-up recommendations.
  • Assist with chart preparation, patient record review, and documentation organization before patient encounters.
  • Collaborate with providers and clinical teams to clarify documentation requirements and facilitate timely completion of medical records.
  • Support quality reporting initiatives, including documentation requirements related to MIPS and other quality programs.
  • Monitor documentation timelines to ensure records are completed within required deadlines.
  • Maintain effective communication with providers, facility staff, and internal team members through approved communication channels.
  • Provide cross-coverage support for fellow Medical Scribes during PTO, leaves of absence, or other operational needs to ensure continuity of provider support.
  • Collaborate with team members to maintain efficient workflows and smooth transitions during coverage periods.
  • Safeguard patient information and maintain strict compliance with HIPAA regulations.
  • Effectively manage remote work responsibilities, including maintaining reliable internet connectivity, a professional work environment, and prompt communication.
  • Travel to assigned skilled nursing facilities as needed to support onsite provider rounding.
  • Perform additional duties as assigned in support of organizational objectives.
QualificationsRequired
  • High school diploma or equivalent.
  • Strong knowledge of medical terminology and healthcare documentation practices.
  • Excellent written and verbal communication skills.
  • Exceptional attention to detail and organizational abilities.
  • Ability to work independently, manage competing priorities, and meet deadlines in a hybrid work environment.
  • Professional demeanor and ability to interact effectively with providers, facility staff, and clinical teams.
  • Ability and willingness to travel to assigned skilled nursing facilities as needed.
Preferred
  • Associate's or Bachelor's degree.
  • Previous experience as a Medical Scribe, Clinical Documentation Specialist, or in a healthcare administrative support role.
  • Experience in wound care, skilled nursing, long-term care, geriatrics, or post-acute care settings.
  • Experience using EMR systems and healthcare documentation platforms.
  • Familiarity with quality reporting programs, including MIPS documentation requirements.
What We Offer
  • Competitive pay of $24.00 per hour
  • Bi-weekly payroll
  • Hybrid work environment with both remote and onsite opportunities
